Missed Lessons: • There will be no refund or make-up class if a student misses a lesson. • In the event that the instructor must miss a class, that class will be made up by the instructor, or credited toward the student’s next course. Manner: • Students are not allowed to bring gum, food or drinks to their lesson. • Students must be on time for their lesson. • No parents are allowed in the classroom, or with written permission from the school office. • Parents are responsible for any damage to merchandise which may occur as a result of their children. • Parents need to escort and pick up students from the classroom. Photo Release: I understand that as part of our child’s participation in the EES, photos, videos, electronic images, audio recordings and quotations of my daughter/son may be taken for use in publications and reports about the program. We grant permission to use such materials of the program. Waiver of Liabilities: I agree with the foregoing Enlighten Enrichment School policy, and I give permission for my child, (Please print your child’s name here) _________________________, to participate in Enlighten Enrichment School and its elective classes. I agree to indemnify and hold harmless Union School District and Enlighten Enrichment School, their officers, directors, staffs, teachers, volunteers and service provides from all liabilities and claims, of whatever nature, with regard to my child’s participation in Enlighten Enrichment School.
